CITY OF LOCKPORT HEALTH INSURANCE ENROLLMENT FORMS

For open enrollment, please print the appropriate form, fill out, and submit to Ashley by October 27, 2023. Please note that current employees do not need to resubmit their health insurance enrollment application during open enrollment each year, rather only in the event of an opted plan change (your current benefit election auto-renews).

HIGHMARK BCBS OF WNY HEALTH INSURANCE ENROLLMENT INFORMATION

For open enrollment, please print the appropriate form, fill out, and submit to Ashley by October 27, 2023. HIGHMARK BCBS OF WNY - MEDICARE ADVANTAGE

PRINCIPAL

​

NEW – Dental and vision insurance benefits are newly offered by Principal and can be added during open enrollment (will take effect January 1, 2024). To elect this benefit, please print the appropriate form, fill out, and submit to Ashley by October 27, 2023. This benefit can also be elected at point of hire and/or during a qualifying life event. Please note that employees can elect this benefit without electing for City-sponsored health insurance, and can also choose to elect only one of the two (or both) benefits.

PRO-FLEX ADMINISTRATORS

​

For annual FSA enrollment during open enrollment, please print the appropriate form, fill out, and submit to Ashley by October 27, 2023. Please note that this election must be done every year and does not carry forward automatically to the 2024 year. This benefit can also be elected at point of hire. PENSERV

Penserv offer deferred compensation plans which can be elected anytime (does not have to be during open enrollment, hire, or qualifying life event). To do so, please contact Jeffery McCabe at jmccabe@centerbridgepg.com or at (716) 626-2518.

AFLAC offers cancer, accident, hospital, and short-term disability coverage. This benefit can only be elected during open enrollment. To do so, please contact Jarrett Pace at jarrett_pace@us.aflac.com.

COLONIAL LIFE

Colonial Life offers cancer, accident, and short-term disability coverage. This benefit can only be elected during open enrollment. To do so, please contact Lou Croce at Lou@landsagency.com or at 716-828-4400 extension 105.

NEW YORK LIFE
